A passport is expected to be a form of identification issued by the government, which states the holder’s country of origin as well as other vital personal information of the bearer. Report Lost/Stolen ID

However, you must first alert the US State Department for the loss or damage of your travel document. Do not fret about this because it is a simple process. You just need to fill out a DS-64 form and submit it to the department. To get the form, all you need do is to visit a passport agency approved by the government.

DS-11 Form

When you are ready to begin your application process, you will have to fill out another form. It is called the DS-11 form. You will need to legibly fill it out with black ink and submit valid evidence of a US citizenship with it. Photocopies of the required documents to back up your US citizenship are acceptable for submission in place of the originals.

In addition, you will also need to provide any undamaged travel document that was issued to you in the past and one of a set of documents considered valid. You can also be required to submit your birth certificate or a consular report of your birth abroad. When that is not within your reach, you can submit a naturalization or citizenship certificate as an alternative.

Other Documents Needed

You may also be required to submit one of these forms of identification in addition to the ones already mentioned above. Your driver’s license, military ID card or government ID card may be part of these required documents. After that, do not forget to include 2 photos (about 2 by 2 inch size) taken within 6 months of the date of your application. You must be dressed in proper clothing, face forward with a white or off white background.

Applying for a U.S. passport replacement is not as difficult as you think. All you need do is to make sure you provide all the needed documents in the exact forms required. Don’t forget to bring your DS-11 form with your application. You also need to bring the other forms of identification required, your photos and a means of payment.
